Provide the full street address, city, and county for each application you are requesting. Include which part of the application you wish to receive. Applications may consist of a Part 1, Part 2, and Part 3 application (narrative), photos, and drawings. Photographs are only provided if specifically requested. Drawings larger than 11x17 cannot be reproduced. These materials contain sensitive information that will be redacted prior to duplication.
