Case History Online Reservation Form

  • To allow for fair distribution fo the Case Histories, only one case is permitted at a location at a time.
  • All Case Histories are available on a first request, first reserve basis upon receipt of a mailed request form. Applications will be accepted after August 1st prior to each school year.
  • Each Case History is loaned for a 12-day period only. On Thursday, the 1st day, it will be shipped UPS (or can be personally picked up). On Monday, the 12th day, each case is to be recieved from UPS (or can be personally returned).
  • The borrower wil prepay a nonrefundable charge of $7.00* to offset each case's shipping expense. Each case is to be returned at the borrower's cost. Cases may be picked up and returned at no cost.
  • All Case Histories are due on the date stated on the confirmation. If a case is late, the borrower will be accessed a late fee of $2.00 per day. Abuse of return date will forfeit shipment of future cases.
  • *Fees must be sent by mail within 5 days of this request.
