- Schedule email according to content
- Proper Usage of "No RC-3 Required"
- Responsible Official must sign forms
- Include a contact phone number so that we may notify you if there
are records that OHS would like to preserve
- The Records Commission Approval date is that which is listed on
the RC-1 or RC-2 containing those records. It is not the date that
the RC-3 was approved.
- If the records are being transferred to another media, list that
in the appropriate column
- Mail AT LEAST 15 working days prior to the scheduled destruction
date.
