Application for One-Time Disposal (RC-1)

Schedule of Records Retention and Disposition (RC-2)

- Do not include dates of the records series.
- Include address and contact information at top of form.
- Schedule email according to content.
- Proper Usage of "No RC-3 Required"
Certificate of Records Disposal (RC-3)
- Responsible Official must sign forms.
- Include a contact phone number so that we may notify you if there
are records that OHS would like to preserve.
- The Records Commission Approval date is that which is listed on
the RC-2 containing those records. It is not the date that
the RC-3 was approved.
- Remember an RC-3 only needs to be used for records series marked as needing review by OHS-LGRP on an RC-2 approved by your records commission after September 29, 2011. If you have an older RC-2, please send in an RC-3 for all records series on your RC-2.
- If the records are being transferred to another media, list that
in the appropriate column.
- Mail at least 15 business days prior to the scheduled destruction
date.
