When Section 106 project submissions to the Ohio Historic Preservation Office are accurate and complete, the review process
is faster and more productive for everyone involved.
Our new Project Summary Form is an interactive checklist for Section 106 submissions that is designed to help agency
officials organize submissions and streamline reviews by the Ohio Historic Preservation Office. This form and other necessary
documentation forms are available for download by clicking on the links below. Please download and save a copy of each
file on your local computer for individual project use.
The Project Summary Form is required for most routine project submissions to OHPO.
(This Project Summary Form is NOT intended for use in any wireless communications projects submitted for review under the
authority of the Federal Communications Commission. Those projects must continue to use the forms provided by the FCC at
http://wireless.fcc.gov/siting/npa.html).
