Registration or Purchases via Purchase Order
Agencies, organizations and other groups who wish to use a purchase order to pay for
Ohio History Store items and workshop or event registration are required to fill out a Request for Sales
Tax Exemption and/or PO Request Form.
The requestor is to submit the completed purchase order request form and a copy of the purchase order
to the Ohio Historical Society Accounting Office by Fax at (614) 297-2411 or by e-mail to
taxexempt@ohiohistory.org. Please call (614) 297-2399 for
questions regarding the purchase order request form.
The requestor will receive a user name and password from the Ohio Historical Society Accounting Office for the
Ohio History Store account within 1-3 business days after receipt of paperwork and can then proceed to register
or make purchases via purchase order.
