A checklist to help you keep track of the elements needed for a successful grant submission.

Below is a checklist that will help you keep track of all the important elements needed for a successful grant submission.

  • All sections completed. Areas not applicable marked "N/A"
  • Budget Form completed and math double checked
  • Match Summary completed and math double checked
  • Budget Narrative provided to explain complex or "other" categories
  • If applicable, Construction Services Budget Form completed
  • Signatures
  • Attachments
    • IRS letter of determination (if applicant is a 501c organization)
    • Most recent IRS Form 990 (if applicant is a 501c organization)
    • Work schedule form (Project Narrative section)
    • Resumes of pre-selected project personnel (Project Narrative section)
    • Budget Form, including Budget Form worksheet, Match Summary worksheet, and Construction Budget Form worksheet (Budget section)
    • Cost estimates for major work items (consultant, construction) (Budget section)
    • Signatures of Authorization form
